New Hampshire lacks a coordinated, real-time system for connecting people to services
From "Welcome to The New Hampshire Forum!"
Go to the project
People in New Hampshire face a fragmented system of healthcare, mental health, financial, housing, and community services. While resources exist, they are not well connected, making it difficult for individuals and providers to see the full picture or respond quickly. As a result, people fall through the cracks and outcomes suffer. We are trying to solve modern challenges with disconnected systems. New Hampshire can lead by creating a more coordinated, real-time approach to supporting its residents.
